Where to Find Us in Phoenix
The address, the hours and the parking, plus the small things nobody puts on a contact page: which number on the door, what to do when you get here, and who picks up when you call.
The clinic, in facts.
One location, one phone number, one inbox. Everything below is current.
Finding the door.
The address is a suite address, so the number that matters once you are out of the car is the suite: 111. Put the whole line into your maps app, 4225 E Windrose Dr Ste 111, rather than the street on its own. That brings you to the right building instead of the right block.
Parking is free and it is by the door. There is no garage to spiral through, no ticket to get validated, and no level number to memorise on the way in so you can find your car again on the way out.
If you are circling and still cannot see it, stop and call (602) 755-3138. Somebody will pick up and talk you in. It is an ordinary call for us to get, and nobody here minds making it.
What to expect when you arrive.
Give yourself a few extra minutes on the first visit only. There is a short intake to fill in and a Notice of Privacy Practices to read, and getting that out of the way at the start is what lets your evaluation have its full 60 minutes rather than the back half of them.
The people who call ahead to ask about parking are usually the same ones who read the whole plan before agreeing to it. That instinct serves you well here, because the evaluation is a conversation before it is a treatment, and the more you tell us the better the plan gets.
Wear something you can move in. Shorts if it is a knee or a hip, a t-shirt or a tank if it is a shoulder or a neck. Bring any imaging reports you already have and the shoes you actually run, lift or work in. Treatment starts in that first visit, so come dressed for it rather than dressed for a consultation.
What happens when you call or write.
The phone is answered by a person during clinic hours. Outside them, leave a message or send the booking form and it is picked up the next morning. Either way a real person calls you back, usually within two business hours, with the times we actually have rather than a link to a calendar with nothing in it.
Waiting is its own decision, and it is rarely a neutral one. The thing you are calling about tends to reorganise the rest of the week around itself, and by the time it has changed how you sit and how you sleep there is simply more of it to unwind.
One thing worth saying plainly on a contact page, because it is the question we field most: we operate as a cash-pay practice and do not bill insurance directly. You get the rate on the phone, before you book, not on a statement afterwards.

The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
